Faculty

Peter C. Neligan, MB, FRCS(I), FRCSC

Dr. Peter C. Neligan is internationally recognized for his work in plastic and reconstructive surgery, particularly microsurgical reconstruction and flap surgery.

In this program, Dr. Neligan demonstrates:

TUG Flap Breast Reconstruction

with detailed operative anatomy and step-by-step surgical technique.

Otway Louie, MD

Dr. Otway Louie demonstrates two additional reconstructive cases:

Nipple Reconstruction and Fat Grafting

and

Bilateral DIEP Flap Breast Reconstruction

These operative demonstrations add both secondary reconstruction and microsurgical breast reconstruction to the library.

Course Overview

Breast reconstruction after mastectomy can involve several different approaches depending on:

Patient anatomy
Previous surgery
Radiation history
Available donor tissue
Reconstructive goals
Need for unilateral or bilateral reconstruction
Patient preference
Surgeon expertise

This QMP video library focuses specifically on autologous tissue reconstruction and secondary reconstructive refinement through three real operative cases.

Rather than providing a broad lecture-only review, the program allows learners to observe:

Preoperative Assessment → Surgical Planning → Anatomy → Dissection → Reconstruction → Monitoring → Postoperative Dressing

Case 1 – TUG Flap

Peter C. Neligan, MB, FRCS(I), FRCSC

The first case demonstrates breast reconstruction using the TUG flap.

TUG stands for:

Transverse Upper Gracilis Flap

This technique uses tissue from the upper medial thigh as an autologous donor source for breast reconstruction.

TUG Flap Breast Reconstruction

The TUG flap may be considered in selected patients in whom the upper thigh provides suitable donor tissue.

Important concepts demonstrated or reinforced through the operative video include:

Patient evaluation
Donor-site planning
Flap design
Upper-medial-thigh anatomy
Gracilis region anatomy
Vascular pedicle identification
Flap elevation
Recipient-site preparation
Microvascular reconstruction
Flap inset
Donor-site closure

Why the TUG Flap Is Important

Abdominal tissue is commonly used for autologous breast reconstruction, but not every patient is an ideal abdominal-flap candidate.

Alternative donor sites may therefore be important.

The upper thigh can offer a useful reconstructive option in appropriately selected patients.

TUG Flap Anatomy

Understanding donor anatomy is essential.

The operation requires awareness of:

Gracilis muscle
Upper medial thigh
Adductor region
Vascular pedicle
Surrounding soft tissues
Donor-site contour

The operative demonstration helps viewers relate anatomic landmarks directly to flap harvest.

Flap Design

Appropriate design influences:

Available tissue volume
Pedicle access
Scar location
Donor-site closure
Final contour

The surgical markings and operative sequence help demonstrate how planning translates into flap harvest.

Flap Harvest

Autologous reconstruction requires meticulous dissection to protect the vascular supply.

Important principles include:

Identification of relevant anatomy
Controlled dissection
Protection of the pedicle
Hemostasis
Tissue handling
Preparation for transfer

Microsurgical Reconstruction

Free-flap breast reconstruction depends on successful microvascular blood flow.

The surgical videos demonstrate microsurgical reconstruction and include use of a coupler device for anastomosis.

Coupler-Assisted Anastomosis

A vascular coupler is commonly used in microsurgery, particularly for selected venous anastomoses.

The operative footage allows viewers to observe how the device is incorporated into a real reconstructive case.

Flap Perfusion & Monitoring

Flap viability is a central concern in microsurgical reconstruction.

The program demonstrates monitoring using the SPY system.

Perfusion assessment can help the reconstructive team evaluate tissue blood flow intraoperatively.

Case 2 – Nipple Reconstruction and Fat Grafting

Otway Louie, MD

The second case combines:

Nipple Reconstruction + Autologous Fat Grafting

This is particularly relevant to secondary breast reconstruction and aesthetic refinement following the primary reconstructive procedure.

Nipple Reconstruction

The nipple-areolar complex contributes significantly to the final appearance of the reconstructed breast.

Secondary reconstruction may be used to improve:

Projection
Shape
Symmetry
Breast contour
Overall aesthetic completeness

The operative video demonstrates practical surgical steps involved in nipple reconstruction.

Secondary Breast Reconstruction

Breast reconstruction often occurs in stages.

After the principal breast mound has been created, patients may require secondary procedures to refine:

Symmetry
Contour
Projection
Local tissue irregularities

Nipple reconstruction and fat grafting are examples of these secondary reconstructive procedures.

Autologous Fat Grafting

The same operative case demonstrates:

Fat harvest
Fat processing
Fat preparation
Fat injection

This allows learners to observe the complete workflow rather than only the injection portion.

Fat Harvest

Autologous fat grafting begins with obtaining adipose tissue from an appropriate donor site.

Technical considerations include:

Donor-site selection
Harvest technique
Tissue handling
Minimizing unnecessary trauma

Fat Processing

Harvested fat requires preparation before grafting.

The video demonstrates processing as part of the operative sequence.

Processing techniques aim to create material suitable for controlled reinjection.

Fat Injection

Fat is then placed into the recipient breast tissue.

Important principles include:

Controlled placement
Small-volume distribution
Appropriate tissue planes
Contour assessment
Symmetry

The goal is typically not simply to inject a large volume into one location, but to distribute grafted tissue according to the reconstructive objective.

Role of Fat Grafting in Breast Reconstruction

Autologous fat grafting can be useful for selected secondary refinements such as:

Contour improvement
Volume correction
Filling localized depressions
Improving transitions
Correcting asymmetry

The exact indication depends on the patient’s reconstruction and anatomy.

Case 3 – Bilateral DIEP Flap Breast Reconstruction

Otway Louie, MD

The third case demonstrates bilateral DIEP flap breast reconstruction, one of the best-known forms of microsurgical autologous breast reconstruction.

What Is a DIEP Flap?

DIEP stands for:

Deep Inferior Epigastric Perforator

The DIEP flap uses:

Lower abdominal skin
Lower abdominal fat
Perforating vessels

for breast reconstruction while aiming to preserve the rectus abdominis muscle.

DIEP Flap Principles

The operative procedure requires detailed knowledge of:

Abdominal wall anatomy
Deep inferior epigastric vessels
Perforator anatomy
Rectus muscle
Anterior rectus sheath
Recipient vessels
Microsurgical anastomosis

Bilateral Breast Reconstruction

In bilateral reconstruction, both breast mounds are reconstructed during the procedure.

This creates additional considerations involving:

Flap distribution
Recipient vessels
Operative sequencing
Symmetry
Microsurgical workflow
Donor-site closure

The QMP case provides direct visualization of these technical steps.

Perforator Dissection

DIEP flap harvest requires careful dissection around the perforating vessels.

The surgeon must balance:

Adequate Pedicle Dissection ↔ Preservation of Abdominal Wall Structures

This is one of the defining technical aspects of DIEP surgery.

Muscle Preservation

A central concept of the DIEP approach is preservation of the rectus muscle compared with techniques that incorporate more muscle tissue.

Understanding the relationship between:

Perforators
Rectus muscle
Deep inferior epigastric vessels

is therefore critical.

Recipient-Site Preparation

Once the flap is harvested, appropriate recipient vessels must be prepared for microsurgical transfer.

The process requires:

Exposure
Vessel preparation
Appropriate positioning
Microsurgical planning

Microvascular Anastomosis

The flap’s blood supply is re-established through microsurgical anastomosis.

The operative library demonstrates these steps and includes the use of a coupler device.

This is one of the strongest educational features of the video program because learners can observe real microsurgical workflow rather than only textbook diagrams.

SPY System for Flap Monitoring

The microsurgical reconstruction videos also demonstrate the SPY system for flap perfusion assessment.

This gives learners an opportunity to observe how intraoperative perfusion technology can be incorporated into reconstructive surgery.

Flap Inset

Once vascular flow has been established, the tissue is shaped and positioned to create the reconstructed breast.

Important considerations can include:

Breast shape
Projection
Contour
Symmetry
Tissue distribution

Donor-Site Closure

The abdominal donor site is then closed following flap harvest.

Successful reconstruction involves attention to both:

Breast Reconstruction + Donor-Site Outcome

Surgical Anatomy

One of the main strengths of the QMP video library is its frequent identification of anatomical structures during surgery.

Relevant anatomy includes:

Thigh

Gracilis region
Upper-medial-thigh tissues
Flap vascular supply

Abdomen

Rectus abdominis
Rectus sheath
Perforating vessels
Deep inferior epigastric system

Chest / Breast

Recipient vessels
Mastectomy site
Reconstructed breast pocket
Soft-tissue planes

This makes the videos valuable for trainees learning how textbook anatomy appears during actual surgery.

Preoperative Presentation

Each case begins with preoperative presentation.

This allows the learner to understand:

Why the reconstruction was selected
Relevant patient anatomy
Reconstructive objectives
Operative planning

Watching the decision-making context before the operation makes the technical demonstration more clinically meaningful.

Step-by-Step Operative Technique

The cases are organized around the actual surgical procedure.

Learners can follow:

Planning → Marking → Incision → Dissection → Flap Harvest → Transfer → Anastomosis → Inset → Closure

depending on the specific operation.

Postoperative Dressing

The cases continue through postoperative dressing rather than ending immediately after reconstruction.

This provides a more complete view of the operative workflow.

Microsurgical Breast Reconstruction

Two key areas of this library make it particularly relevant to microsurgical trainees:

Autologous free-flap reconstruction
Microvascular anastomosis

Successful microsurgical breast reconstruction depends on coordination of:

Patient selection
Donor-site anatomy
Flap selection
Vessel dissection
Recipient-vessel preparation
Anastomosis
Perfusion assessment
Flap inset
Postoperative monitoring

TUG vs. DIEP – Different Donor Sites

The program provides exposure to two different autologous donor regions.

TUG

Upper medial thigh

DIEP

Lower abdomen

This provides useful visual comparison between two different reconstructive strategies.

Selecting an Autologous Flap

Flap selection depends on individual patient factors.

Considerations can include:

Available donor tissue
Previous abdominal surgery
Body habitus
Reconstructive volume requirements
Bilateral vs. unilateral reconstruction
Previous radiation
Patient preference
Donor-site considerations

The library should be viewed as procedural education rather than a claim that one flap is optimal for every patient.

Nipple Reconstruction as a Secondary Procedure

Breast reconstruction frequently involves more than creation of the primary breast mound.

Secondary procedures can play an important role in achieving a more complete reconstructive result.

The nipple reconstruction case demonstrates one stage of this process.

Fat Grafting as a Refinement Tool

Fat grafting can complement breast reconstruction by helping address localized contour or volume irregularities.

The QMP case is particularly useful because it demonstrates the full sequence:

Harvest → Process → Inject

rather than only discussing fat grafting conceptually.

Surgical Pearls

Throughout the video library, learners can focus on practical details such as:

Anatomical landmarks
Flap orientation
Tissue handling
Pedicle dissection
Microsurgical preparation
Anastomosis
Perfusion assessment
Fat handling
Flap inset
Closure
